Return Material Authorization (RMA) Request Form  
  • Complete all form fields to expedite the repair.
  • An RMA number will be e-mailed or faxed after review of this completed form.
  • Write the RMA number on the outside of the return shipping box.
  • Return product to DSE at 17491 North 93rd Street, Scottsdale, AZ 85255-6324.
  • Allow 4-6 weeks for repair and/or return.
  • Expedite Repair may be available for an additional charge.
  • Customer is responsible for return freight to DSE.
  • Domestic *Warranty Repairs. RMA returned to Customer without freight charges, shipped ground.
  • International *Warranty Repairs. RMA returned to Customer may incur freight charges.
  • Nonwarranty Repairs. Recommended: RMA returned on Customer's shipper's account; Prepaid shipping by DSE will incur a handling charge.
  • Storage Fees. If within 45 days of DSE bid quote submission Customer does not respond with a qualified Purchase Order (or Credit Card payment), RMA unit will incur a $25.00 storage fee.
  • Warranty/Nonwarranty Repairs with 'No Trouble Found' Findings. Will incur return freight charges and are subject to bench test fee.

Nonwarranty Repairs: DSE Standard Nonwarranty process is to notify Customer with a written repair estimate prior to any repair. Accepted Forms of Payment: Credit Card or qualified Purchase Orders only. Warranty and Nonwarranty Repairs with 'No Trouble Found' Findings: will incur return freight charges and are subject to bench test fee. Storage Fees: if Customer does not respond to repair bid quote with a qualified Purchase Order (or credit card payment) within 45 days of DSE bid quote submission, RMA will incur a $25.00 storage fee.
